50次卡（ 人民币 10888 元），会员权益：（送 节课）（ 赠品 ）- 以上赠送事项不能转贈他人及不可兑换现金及其他产品，课程及服务会员卡有效期：从第一次预约课程起 个月内有效。
会员资格：
1、会员需确保自己的身体处于健康状态，没有任何不适运动的疾患损伤。
2、会员的最低年龄为16周岁（进行少年项目除外，但需有家长或监护人签字）。
3、本公司将收取一次性入会费（ 元），购买会员卡后即取得入会资格。会员卡办理时已经享受优惠，一经售出，不予办理退卡或退款。
4、会员卡是证明会员资格的凭证，仅供本人使用，不得私自转让；若会员却因自身原因需转卡的，需征得本公司书面同意并收取购卡总价的20%作为手续费。
5、报停：如会员因个人原因不能正常参加锻炼，会员可申请报停，每个会员可享受一次报停服务，报停会收取相应服务费，报停必须以书面形式提前办理并取得本公司书面同意方可生效。
6、会员卡过期：会员卡过期即所有会员权益失效。会员卡可续期，每月收取（¥ 200 ）作为延续会员费用，一经付费权益可在第二天生效
会员义务：
1、会员必须出示自已的会员卡才能进入本场馆；如果忘记携带，应凭借会员卡号和本人身份证进入本场馆。
2、本场馆的营业时间为：10：00-22：00。本公司有权根据特殊需要适当调整营业时间并店内公示。
3、进入本场馆会员应当举止文明、语言规范、不大声喧哗；会员、嘉宾或访客不得对本场馆的员工进行人身攻击。
4、本场馆严禁饮酒，吸烟和吸毒；不得携带宠物进入本场馆。
5、因不当的操作导致器材损坏需照价赔偿。场馆内所有物品不得损坏，如有损坏照价赔偿。
场馆设施及服务：
1、因经营管理需要，本公司有权调整、增减部分项目，该行为不视为违约，且在店内公示后即视为通知，无需另行报告给会员个人。
2、因器械、设备（设施）检修、维护，本场馆有权在某一时间段对某一项目或某类项目采取停用或限用措施。
3、会员在使用配有安全带的器械时，必须做好安全措施；否则本公司对个人原因造成的意外和损伤不负责任。
4、本公司不提供任何有关医疗、医学、医药方面建议。若因自身身体存在疾患将有可能给您带来伤害，您需自己承担责任，该情况的发生与本公司无关。
免责条款：
出现下列情形的，本公司不予承担任何责任:
1、遇不可抗力（如战争、自然灾害等）造成场馆营业终止或会员会籍不能继续，致使会员遭受损失的。
2、会员所受损害是因其自身故意或过失造成的；会员所受损害是本场馆工作人员以外的任何第三方的故意或过失行为导致的。
3、未交由本场馆保管而由会员或会员随同人员个人保管的贵重物品发生毁损、灭失、遗失的；
4、未听从本公司工作人员指导，擅自使用或违反操作规程使用器械、设备造成自身受伤的，由自身负责；造成他人受损或本公司财产毁损的，其本人应承担全部赔偿责任，对此本公司不承担任何责任。
安全提示：
1、会员在锻练前，应先做必要的热身练习，以免受伤。
2、过度锻炼及违规锻炼均有受伤的可能，所以您在运动前应对自己的身体情况进行判断，并保持运动强度和时间的适当。
3、本场馆内任何运动项目及器械设施均有严格的操作方法和程序，请务必在专业人员的指导下进行操作，如有违背致使人员受损，本公司不承担任何责任。
特别声明：
本公司对该协议约定的内容有最终解释权，同时为更好地服务会员之需要，本公司有权对相关内容进行修改，且修改后的条款自网上通告或在场馆显著位置公示后，即对全体会员产生约束力。
会员承诺：
1、本人保证所提供的入会资料及个人信息真实有效； 2、本人身体健康且没有本协议约定的不适合进行运动的疾病； 3、本人已阅读、理解并同意上述条文

Terms and Conditions – JUMPLIFY BUNGEE WORKOUT THEATRE

1. The Participant hereby accept and agree to be bound and comply with terms and condition set forth by JUMPLIFY BUNGEE WORKOUT THEATRE, including any terms and condition set into effect after the date which this application form has been signed by the Participant, should such terms and condition benefit the participant, especially for the Participant’s safety.
参与者同意并接受JUMPLIFY BUNGEE工作坊所设立的条款和条件，并加以完全遵守。参与者签署申请表后JUMPLIFY BUNGEE工作坊所设立生效的条款和条件，若有利于参与者，特别是为了参与者的安全考虑，对此条款和条件，参与者完全同意并遵守。
2. The Participant warrants that the Participant had provided truthful answer, regarding to health questionnaire, to JUMPLIFY BUNGEE WORKOUT THEATRE. The Participant shall disclose its medical information to JUMPLIFY BUNGEE WORKOUT THEATRE as per requested.
参与者保证已向JUMPLIFY BUNGEE工作坊提供了有关健康问询方面的真实回答。参与者应按照要求向JUMPLIFY BUNGEE工作坊披露有关医疗健康信息。
3. The Participant acknowledges that the use of JUMPLIFY BUNGEE WORKOUT THEATRE’s utilities, tools, or equipment, may cause bodily injuries or damage to the Participant, the Participant voluntarily engage to such risk by free will. Should the Participant die or acquire injuries during the use of such utilities, tools or equipment, caused by any fault not attributable to JUMPLIFY BUNGEE WORKOUT THEATRE, the Participant accept and agree that JUMPLIFY BUNGEE WORKOUT THEATRE may elect to be held exclusion of liability, in so far as admissible by law. JUMPLIFY BUNGEE WORKOUT THEATRE warrant that any service provided to the Participant, shall be done with reasonable skill and care.

4. The Participant shall keep its own mental and physical limits in consideration. JUMPLIFY BUNGEE WORKOUT THEATRE may elect to be held exclusion of liability to any harm, fatality or damage caused by such reason.
参与者应充分考虑自己的心理和身体上的限制，超出心理和身体所限造成的任何伤害、死亡或损害，JUMPLIFY BUNGEE工作坊概不负责。
5. JUMPLIFY BUNGEE WORKOUT THEATRE may assign any of its rights and obligation under this Terms and Condition or any Subcontract without prior notice to the Participant and the Participant accept and agree that any obligation of JUMPLIFY BUNGEE WORKOUT THEATRE can be assigned to a third party.
JUMPLIFY BUNGEE工作坊可以在事先不通知参与者的情况下，将本条款和条件或任何附属合同项下的权利和义务转让给第三方，参与者对此同意并接受。
6. The failure of JUMPLIFY BUNGEE WORKOUT THEATRE to enforce its rights and obligation, at any time or any reason whatsoever, shall not be construed as a waiver of its rights and obligation to the Participant.
JUMPLIFY BUNGEE工作坊不能执行其权利与义务，不论任何时间或任何原因，都不应视为其对参与者权利和义务的放弃。
7. The Participant agrees to allow JUMPLIFY BUNGEE WORKOUT THEATRE to collect Participant’s information and medical information of the Participant for the benefit of the Participant’s safety and to promote activities and sale promotion of JUMPLIFY BUNGEE WORKOUT THEATRE or any of its affiliates.
为了参与者的安全利益，参与者同意JUMPLIFY BUNGEE工作坊收集参与者的信息和医疗健康信息，并允许JUMPLIFY BUNGEE工作坊或其附属机构进行活动改进和促销活动。
8. In case of the Participant’s age is below 20, the Participant shall acquire the consent to be bounded by any terms and condition, set forth between the Participant and JUMPLIFY BUNGEE WORKOUT THEATRE, from parent(s)/guardian(s) and including the acceptance of liability to any damage arises under this terms and condition.
参与者和JUMPLIFY BUNGEE工作坊之间设立的条款和条件，对参与者具有约束力，如果参与者的年龄低于20周岁，应获得其父母或监护人的同意，该同意包含对本条款和条件下所引发的任何损害赔偿责任的接受。
9. The Participant agrees that JUMPLIFY BUNGEE WORKOUT THEATRE can use lecture videos of the Participant for commercial promotion for free. 参与者同意JUMPLIFY BUNGEE工作坊可以无偿使用其上课视频作为商业宣传。
10. The Participant accept and agree not to copy, replicate or mimic any instructional equipment under the course of JUMPLIFY BUNGEE WORKOUT THEATRE and the Participant shall not use any knowledge or technique to use such instructional equipment to compete with JUMPLIFY BUNGEE WORKOUT THEATRE. Instructions under the course of JUMPLIFY BUNGEE WORKOUT THEATRE were made only for health and entertainment purpose. The Participant cannot use for instructing third parties or commercial purpose.
参与者承诺不会复制、模仿或仿制JUMPLIFY BUNGEE工作坊课程中的任何教学指导设备，参与者同意不会利用任何知识和技能使用JUMPLIFY BUNGEE工作坊课程的教学指导设备与JUMPLIFY BUNGEE工作坊发生竞争。JUMPLIFY BUNGEE工作坊课程中的教学指导仅是为了健康和娱乐的目的，参与者不能使用该教学指导为第三方或商业之目的。
11. The Participant accept and agree not to teach/ convey/ show or replicate instructional equipment mentioned in clause 9 or commit any act to pass on knowledge acquired from the training/ instructions from JUMPLIFY BUNGEE WORKOUT THEATRE and/or other institute procured by JUMPLIFY BUNGEE WORKOUT THEATRE to third party, the Participant also commit not to compete with JUMPLIFY BUNGEE WORKOUT THEATRE, whether directly or indirectly, without written consent from JUMPLIFY BUNGEE WORKOUT THEATRE. Should the Participant commit any act as aforementioned, that may cause damage to JUMPLIFY BUNGEE WORKOUT THEATRE, the Participant accept and agree to compensate as claimed by JUMPLIFY BUNGEE WORKOUT THEATRE, the maximum liability shall not exceed RMB1,000,000 (RMB One million only)
参与者承诺不会教授/传递/展示或复制第9条所提及的教学指导和设备。从JUMPLIFY BUNGEE工作坊教学和训练中及从JUMPLIFY BUNGEE工作坊采购商处获取的知识，参与者同意不会采取任何行动传授给第三方。没有得到JUMPLIFY BUNGEE工作坊的书面同意，参与者也不会致力于与JUMPLIFY BUNGEE工作坊发生竞争，不论直接的或间接的。如果参与者未履行承诺，发生了以上所提及的任何行为，参与者接受并同意对JUMPLIFY BUNGEE工作坊主张的损失予以赔偿，赔偿责任的最大限额应不超过100万元人民币。
