会员协议
第一章总则 General Rules
第一条：本俱乐部名称 Club name_____ 三毛健身 未来店 中山店_____(以下简称“本俱乐部”)
本俱乐部地址：Club Address____ 汉兴路三毛未来大厦5-6楼 中山路三毛时代3楼_____
第二条：本俱乐部与会员均有义务遵守此协议，并可依据政府法律调整和行业规范变更等情况，对本协议进行修改，修改的内容将在本中心内部张贴十五（15）日作为对会员的告知。
The club and members are obliged to observe the rules, which may be modified according to the changes of governing laws and industry standards. And all the modification shall be posted publically in the club for at least 15 days in order to make the members well informed.
第二章经营及管理 Operation and Management
第三条：本俱乐部所有经营、管理等营运规章制度依本公司董事会决议或授权行使。
The operation, management and service rules of the club are all based on decisions made by and authorizations given by the board of directors.
第三章会员之权利与义务 Rights and Liabilities of Members
会员的权利 Rights of Members
第四条：会员根据自身会籍类型的不同，享有使用既定本俱乐部服务与各种由本俱乐部提供的专业服务的权利。“专业服务”将包括但不限于初始体质测评、后续体质测评（另行付费）、健康顾问、各类运动器材使用、各类课程参加等，视本俱乐部情况而定，本公司和本俱乐部有权不时颁布、修改服务及课程内容。会员有权自行选择当时公布的有效的专业服务和课程内容，但是未使用专业服务和|或未参加课程不得减少会员支付的购卡价格（如此协议第六条所定义）。
The members have rights to enjoy the club's established services and other professional services according to their membership types. “Professional services' include but are not limited to preliminary physical assessment, follow-up physical assessment(be surcharged),health consultancy, using the club's fitness facilities, attending different classes and courses etc. The club reserves the right to issue and revise the services and courses in different circumstances. The members is entitled to choose professional services and courses available at that time, but the renouncement of professional services and/or courses does not lead to reduction of membership card price (As regulated in Rule Six)
第五条：会员卡的暂停使用 The suspension of membership card
在一些特别的情况下，例如出差、休假等，会员可将会籍时间临时冻结30天以上、90天以下(不计会籍时间)，但须遵循以下规定：
On some special occasions, such as business trip, vacation, the members can apply for suspension of their membership card for at least 30 days and at most 90 days in accordance with following regulations:
1.会员资格冻结手续必须事前办理，不得事后补办。Members must inform the club in advance of their membership suspension and the application after the suspension will not be accepted.
2.根据卡类型，有挂卡期的会员才能办理会员资格冻结手续。（冻结期限：年卡30天、两年卡60天、三年卡90天）
The membership suspension can be used only when the cardholder has a suspension period, according to the type of membership.（Freeze period: year card 30 days, the two card for 60 days, three years 90 days）
3.会员资格冻结时限到期后，会员资格将自动恢复，并开始计算会籍时间。
After the suspension, membership will be renewed automatically and the membership time will be recalculated.
4.女会员因怀孕可享受连续12个月的会员资格免费冻结。但必须事前持有效证明至会所亲自办理。
The pregnant members can enjoy a free suspension period continuous for up to 12 months. But members must go to club by themselves in advance with effective certificates.
5.会员在冻结期内不得使用俱乐部（包括不得使用私人教练课程）。如果提前使用，会员冻结期即日启动，并视为完整结束。
During the suspension period, members cannot use any equipment or courses (including private coaching). Otherwise, the suspension will be terminated right away.
第六条：会员卡的转让 Membership Transfer
会员会籍转让必须遵循以下规定
The transfer of membership must comply with following regulations:
1.会员卡必须在会籍启动后转让、且只可转让一次、受让者不得再次转让。
Membership only can be transferred after being activated and only can be transferred once. The transferee can not retransfer the membership.
2.转卡须缴200元转卡费，接受人由会员自行寻找。
Turn card must pay 200 yuan fee, to accept people find by the members themselves.
3.被转让人不享有转让人购卡时享有的特殊优惠，包括但不仅限于赠券，更衣柜等。
“购卡合计价格”指会员在获得会员卡时支付的一次性费用，如同此协议文首列明的各项费用的合计。包括会籍服务费、初始制卡费、体质测试券、礼品费、健康顾问等。
The transferees can not enjoy any special preferential treatments entitled to the transferors such as gifts coupons, lockers etc. “the total price of card” means the fee that members pay for the membership card at one time, as the price listed at the head of this agreement, including membership service, initial card-making fee, physical assessment fee, gift fees and health consultancy etc.
第七条：会员享受续会优惠的权利，但须遵循以下规定：
The members enjoy the right of extending their membership at a preferential rate in accordance with following regulations:
续会必须在会籍有效期内进行，否则不享受优惠价格。续卡价格按照当前价格或当前活动优惠价格。
The members must extent their membership before their current membership expires, otherwise they will have to re join the club at the current price level.Renewal card price according to the current price or current activities discount prices..
会员的义务 Liabilities of Members
第八条：会员因特殊情况无法亲自办理相关手续的，可委托他人持委托书及委托人与被委托人的有效身份证件，持会员卡进行办理。
If members cannot go through formalities by themselves because of some special situation, they can entrust another person go through the formalities with Letter of authorization, their ID cards and membership card.
第九条：会员卡仅供本人使用，不得出借他人。
Member cards only used by member himself/herself and can not be borrowed to other person.
第十条：会员进入本俱乐部必须出示会员卡，该会员卡必须拍照且该会员卡在有效期内。
Members should show their current membership cards when entering our club, and the membership card must have member's photo.
第十一条：会员对本俱乐部设施造成损坏，需按价赔偿。
Members should compensate the club if they damage any of facilities in the club.
第十二条：会员购卡后，必须于会员协议签定日七日内启动会籍，七日内如无手动启动，七日后会员系统将默认开始计算会籍试用期。
After buying membership card, member should start membership within the seven days after the day of signing the agreement. The membership if not started by a member within the seven days, it will be started automatically by the club's system.
第十三条：会员卡遗失，会员应及时向本俱乐部报失，并办理补卡手续：20元/张。
The members should report the loss of a membership card to our club in time and reapply for a new card. at 20 RMB/card.
第十四条：为保障安全，除本俱乐部开设少儿课程班外，会员严禁带领16周岁以下的儿童进入本俱乐部。在开设少儿课程时，会员对其子女或由其看管的儿童在本俱乐部的一切行为承担责任。子女或由其看管的儿童在本俱乐部内造成自身和他人人身、财产损失，会员应当承担全部责任，本俱乐部不承担任何责任。
For sake of safety, the members are prohibited to bring children less than 16 years into the club unless the club's courses for children are open. During the children courses, the members must take all responsibilities for all behaviors of their own kids or kids under their custody. If members' own kids or kids under their custody cause loss and damage, the members, instead of the club, should bear all of corresponding responsibilities.
第十五条：为了您锻炼的安全和舒适，本俱乐部建议会员在锻炼时穿着运动鞋与相应的运动服装并不佩戴首饰。
We strongly recommend that our members should better wear sports shoes and appropriate sports clothes and not to wear any jewelry and accessories when they are exercising in the club.
第十六条：由于对设施进行保养、维修等原因，本俱乐部保留暂时关闭该项设施的权利。
The club reserves the right to close any facilities provisionally when they are under maintenance or being repaired.
第十七条：自会员购得会籍资格办理会员卡时，本俱乐部即得收取会籍费、手续费和会员卡费。会员卡的使用年限、使用范围、缴费金额及缴费方式等明细依照会籍种类及费用条款执行。上述费用一旦支付，无论会员是否使用会所的设施设备，都将无法退款。
Upon the time the members have purchased the membership, the club can collect the membership fee, commission charge and card-making fee. The term, range of application, price and payments of membership card will be regulated according to membership types and corresponding conditions. All the fees are can not be refunded, regardless of whether the members use the club's facilities or not.
会员的权利 Rights of Members
第十八条：会员有以下情况之一，可申请办理退卡手续：
（1）会员死亡或宣告死亡（须提供相关证明材料）；
（2）办理移民手续的（须提供相关证明材料）；
如满足退卡条件申请退卡的，本俱乐部有权扣去消费时间的金额、消费项目款项，如刷卡消费需扣除手续费。
The members are allowed to quit the membership under circumstances as following:
1. The members are perished or being announced perished(valid certificated must be provided)
2. The members immigrate to other countries(valid certificated must be provided)
Apply for card such as card conditions, this center shall have the right to buckle to the amount of consumption of time and money in the form of consumption, such as brush calorie of consumption to deduct fees.
第十九条：会员在本俱乐部使用设施导致自身或他人受伤、死亡；若因本中心原因导致，由本俱乐部负相关责任；若因会员原因所致，由会员本人负全部责任。
If an accident, member's injury or death occurs when using the facilities and it is the fault of the club then the member will be compensated under Chinese law. If injuries or deaths result because of the member's misuse or own fault then there will be no compensation.
第二十条：会员因违反此会员协议规定，经本中心交涉仍不履行，即视为违约。本俱乐部有权终止会员资格、并不予退还剩余会员费用作为违约金。会员不得以任何形式在会所内从事与本会所经营服务有冲突的盈利与非盈利行为，一经本俱乐部单方确认，有权终止会员会籍服务。
If a member does not or refuse to comply with the membership contract, the club can terminate the membership without any refund. Members are prohibited to do any behaviors profitable or unprofitable in the club. If we confirm the circumstance, we are entitled to terminate the membership service.
第四章会员购买私人教练课程 Private Coaching Purchase
第二十一条：会员在会员卡有效期内可以自愿购买私人教练课程
Members can buy PT sessions voluntarily during their current membership period
第二十二条：会员上私人教练课前须就各课程签署私教课时记录卡。
Members must sign in on the PT session record card before the session begins.
第二十三条：购买的私人教练课程均需在会员卡有效期内完成，会员卡到期则所剩课时将自动取消。
All PT sessions must be completed on or before the membership expires. And all sessions will be canceled after the stipulated date.
第二十四条：私人教练课程费用不予退回，亦不可作为抵消任何会员账户余额。课程可转让给阁下所指定的俱乐部会员，但须经中心管理人员批准并填妥课程转让表。
The PT sessions are not refundable and cannot be used to offset any membership account balance. PT Sessions may be transferred to another member nominated by you of the club, but must be approved by the club's management and you have to fill in a transfer application form in advance.
第二十五条：当原指定私人教练无法完成课程时，俱乐部保留更换私人教练的权利。
In the event the original scheduled trainer is unable to conduct a workout. The club reserves the right to provide another substitute certified personal trainer for you.
第二十六条：会员与教练提前24小时预约如不能到场，须提前4小时致电或亲临俱乐部通知有关人员取消当天课程。否则，会员仍须交付该课程相关费用。
A cancellation notice 4-hour in advance is required for any sessions otherwise the member will be still charged for the missed session.
第二十七条：如果会员了解自己身体有任何关于身体方面的原因而导致不能使用本中心的任一器械或运动项目，请事先告知自己的私人教练。否则如果发生身体危险，或达不到会员所希望的目标，本俱乐部不负任何责任。
If you are aware that some of your physical conditions preventing you from using any facilities and training services in the club, please tell your PT in advance. Otherwise, if any dangers occur or you can not accomplish the anticipated targets, the club does not bear any responsibilities.
收银台为俱乐部唯一服务项目支付点，任何私自交易的教练指导性服务，本俱乐部不承担任何连带责任。
The member should only pay at the cashier counter. The club will not be responsible for any training service which traded in private.
第五章更衣柜及私人储物柜的使用管理规定 Locker and private locker
第二十八条：公共更衣柜和私人储物柜为寄存日常用品之用，不得存放现金、电脑、手机、贵重物品及任何食品，禁止存放易燃易爆及非法物品，如存放了贵重物品而丢失，本俱乐部将不负担赔偿责任。
Public lockers and private lockers are used to deposit daily-use articles and cannot be used to Deposit cash, computers, mobile telephones，Valuables and food. The explosives, flammable and illegal articles are strictly prohibited to be stored in any lockers in the club. The club will not be responsible for any loss of valuables in the lockers.
第二十九条：公共更衣室内的更衣柜为公共使用，会员无论任何原因离开俱乐部时，必须打开更衣柜并不得私自占用，本俱乐部有权在会员不在俱乐部时清理更衣柜并无法负责保管其中的物品。
Whenever the members leave the club for any reasons, they must keep the public lockers open. The club has right to clean the public lockers unoccupied and is not responsible for taking care of any articles left in them.
第三十条：会员如长期占用公共更衣柜，本俱乐部有权扣除其会员卡使用时间。
If the members occupy public lockers for long time, the club has the right to cut down the membership time.
第三十一条：会员租用的私人储物柜仅限在租赁协议规定有效时间内使用，会员需在私人储物柜到期前至前台或水吧办理续约手续。如不再续约需清空租赁柜并告知前台。同时会员卡到期，私人储物柜如有未使用月份将自动作废。
Private Lockers can only be used during period stated in the rental contract. Once the period has expired, the members must renew the rental contract at front desk and water bar. If not to renew it, the members have to empty the lockers and inform the front desk. The private locker rental contract will expire once upon the membership expires.
第三十二条：租赁柜到期一周后，如会员不再续约，本俱乐部将对租赁柜进行清理。并无法负责保管其中的物品。
If a rental locker is not vacated one week after the rental contract expires, the club will clear the lockers and the club will not be responsible for taking care of articles inside the lockers.
第三十三条：公共更衣柜手牌需会员保管好，如有丢失收取手牌成本费，私人更衣柜钥匙由会员自行保管，如有丢失收取钥匙成本费。

第三十四条：私人储物柜仅限会员本人使用，不得转借或转租。
Rental lockers are for current members only. It is forbidden to borrowed or rented to other person.
第三十五条：私人储物柜一经租赁后，若因会员单方面原因要求退租，俱乐部将扣除您的所有租金,并将押金无息返还。
Private locker after once the lease, if the member asks to hire, unilateral club will deduct all your rent, and return the deposit without interest..
第六章必须申报的情况和禁止的行为 Situations must be announced and behaviors must be forbidden
第三十六条：患有皮肤病、传染病、心脏病、高血压、糖尿病、肝、肾功能不良及其他不适合本俱乐部活动的疾病患者，入会前须向本俱乐部说明和申报以往病史，因隐瞒上述各类病史入会后在本俱乐部发生意外或加重病情的，后果由会员独自承担，本公司和本俱乐部概不负责。
People who suffer from skin disease, infectious diseases, heart disease and hypertension or any serious illnesses or afflictions must state to the club their health history. If members do not disclose their health history and suffer injury when using the facilities in the club, the club does not take any responsibilities.
第三十七条：吸食毒品者不得入会，因隐瞒吸食毒品而入会的，一经发现立刻取消会员资格，所有费用不予退还，造成的后果由其本人自行承担，本俱乐部概不负责。
Drug addicts and drug users are not welcome to join our clubs. If a member conceals their drug addiction , their membership will be cancelled immediately without refund or compensation. The club will not be liable for any loss.
第三十八条：饮酒者在体内酒精未分解挥发之前不得进入本俱乐部参加各项运动，因饮酒而强行进行活动发生意外的，责任由其自行承担，本公司和俱乐部不负任何责任。进入本俱乐部的人员不准携带和饮用含有酒精的饮料。
Members are not allowed to use the facilities if they are drunk or have consumed large amounts of alcohol. The club accepts no responsibility and is not liable for injuries cause because members are drunk. Anybody is forbidden to bring alcohols into the club.
第三十九条：会员在参加活动后即时将自己的私人物品带离俱乐部，本俱乐部不负保管义务，因会员未能及时将私人物品带走而发生遗失的，本俱乐部概不负责。
Members who take personal belongings into the club are wholly responsible for them. The club takes no responsibility for any loss of belongings within the club.
第四十条：定金保留有效期为五个工作日，过期将不予退还。在交纳定金时候必须指定办卡人员信息，定金不得转让他人使用。
Deposit payments have a validity of five working days. If this time is exceeded and the member has not paid the balance and joined the club, then the deposit can not be refunded. Transfers of deposits are not allowed.
第四十一条：会员需提前声明，身体状况良好，并无任何医学原因、损伤或残障阻碍，不影响使用本俱乐部所有设施及私教训练服务。
Members must state before they join the club they are in good health and are able to use all facilities and join in classes without any ill affects.
第四十二条：会员签署本协议后，本俱乐部不负责就会员使用设施及接受集体训练服务的身体状况及能力向会员提出医学意见。如会员在签署本协议之时或之后有任何健康方面的担忧，应在使用设施及进行私教训练之前咨询专业医生。
The club is not responsible for giving medical advice or suggestions about the member's health. If a member is concerned about their health after joining the club they must consult a medical physician/doctor.
第四十三条：会员在知悉或理应知悉自身存在任何可能阻碍自己使用俱乐部全部设施的健身问题，而仍签署了本协议，即表明会员同意本身的练习活动会因此受限制。然而由于会员选择签署本协议，仍需缴付有关的费用，犹如会员使用全部设施一样。
Members who sign the agreement and pay the membership fee should acknowledge they can use all of the facilities; members who have limitations on certain equipments will not be compensated.
第七章附则 Bylaws
第四十四条：会员不得在俱乐部内进行未经本俱乐部允许的任何收费类活动，包括但不限于对于指导会所内其他会员训练30分钟以上时间的情况。本俱乐部有权停止该会员的会籍期。
Members are not allowed to conduct any business what so ever within the club, especially personal training if members are seen training or guiding other members for longer than 30 minutes, otherwise they will have their membership terminated without refund.
第四十五条：本协议若经涂改或有手写条款，均视为无效，仍以本协议原始印刷文字为准。
Any changes or handwritten notes on the agreement will not be accepted and be invalid. The agreement is not to be changed in any way.
第四十六条：本协议具有约束力，并取代任何口头或书面协定。且法院宣布本协议任何部分无效，本协议其余部分将不受影响并继续有效。本俱乐部因任何原因而未能执行本协定下的任何权利，并不代表本俱乐部将放弃于日后执行有关权力的权利。
This agreement has binding force and substitutes any spoken or written agreement. If court announces that part of this agreement is not valid, other parts will not be affected. If the Club failed to carry out any right under this agreement, it does not mean that the club will give up carrying out these rights in the future.
第四十七条：此协议最终解释权归三毛健身所有。
The interpretation of this agreement shall be owned by the three hairs fitness.

For sake of safety, the members are prohibited to bring children less than 16 years into the club unless the club's courses for children are open. During the children courses, the members must take all responsibilities for all behaviors of their own kids or kids under their custody. If members' own kids or kids under their custody cause loss and damage, the members, instead of the club, should bear all of corresponding responsibilities.
第十五条：为了您锻炼的安全和舒适，本俱乐部建议会员在锻炼时穿着运动鞋与相应的运动服装并不佩戴首饰。
We strongly recommend that our members should better wear sports shoes and appropriate sports clothes and not to wear any jewelry and accessories when they are exercising in the club.
第十六条：由于对设施进行保养、维修等原因，本俱乐部保留暂时关闭该项设施的权利。
The club reserves the right to close any facilities provisionally when they are under maintenance or being repaired.
第十七条：自会员购得会籍资格办理会员卡时，本俱乐部即得收取会籍费、手续费和会员卡费。会员卡的使用年限、使用范围、缴费金额及缴费方式等明细依照会籍种类及费用条款执行。上述费用一旦支付，无论会员是否使用会所的设施设备，都将无法退款。
Upon the time the members have purchased the membership, the club can collect the membership fee, commission charge and card-making fee. The term, range of application, price and payments of membership card will be regulated according to membership types and corresponding conditions. All the fees are can not be refunded, regardless of whether the members use the club's facilities or not.
会员的权利 Rights of Members
第十八条：会员有以下情况之一，可申请办理退卡手续：
（1）会员死亡或宣告死亡（须提供相关证明材料）；
（2）办理移民手续的（须提供相关证明材料）；
如满足退卡条件申请退卡的，本俱乐部有权扣去消费时间的金额、消费项目款项，如刷卡消费需扣除手续费。
The members are allowed to quit the membership under circumstances as following:
1. The members are perished or being announced perished(valid certificated must be provided)
2. The members immigrate to other countries(valid certificated must be provided)
Apply for card such as card conditions, this center shall have the right to buckle to the amount of consumption of time and money in the form of consumption, such as brush calorie of consumption to deduct fees.
第十九条：会员在本俱乐部使用设施导致自身或他人受伤、死亡；若因本中心原因导致，由本俱乐部负相关责任；若因会员原因所致，由会员本人负全部责任。
If an accident, member's injury or death occurs when using the facilities and it is the fault of the club then the member will be compensated under Chinese law. If injuries or deaths result because of the member's misuse or own fault then there will be no compensation.
第二十条：会员因违反此会员协议规定，经本中心交涉仍不履行，即视为违约。本俱乐部有权终止会员资格、并不予退还剩余会员费用作为违约金。会员不得以任何形式在会所内从事与本会所经营服务有冲突的盈利与非盈利行为，一经本俱乐部单方确认，有权终止会员会籍服务。
If a member does not or refuse to comply with the membership contract, the club can terminate the membership without any refund. Members are prohibited to do any behaviors profitable or unprofitable in the club. If we confirm the circumstance, we are entitled to terminate the membership service.
第四章会员购买私人教练课程 Private Coaching Purchase
第二十一条：会员在会员卡有效期内可以自愿购买私人教练课程
Members can buy PT sessions voluntarily during their current membership period
第二十二条：会员上私人教练课前须就各课程签署私教课时记录卡。
Members must sign in on the PT session record card before the session begins.
第二十三条：购买的私人教练课程均需在会员卡有效期内完成，会员卡到期则所剩课时将自动取消。
All PT sessions must be completed on or before the membership expires. And all sessions will be canceled after the stipulated date.
第二十四条：私人教练课程费用不予退回，亦不可作为抵消任何会员账户余额。课程可转让给阁下所指定的俱乐部会员，但须经中心管理人员批准并填妥课程转让表。
The PT sessions are not refundable and cannot be used to offset any membership account balance. PT Sessions may be transferred to another member nominated by you of the club, but must be approved by the club's management and you have to fill in a transfer application form in advance.
第二十五条：当原指定私人教练无法完成课程时，俱乐部保留更换私人教练的权利。
In the event the original scheduled trainer is unable to conduct a workout. The club reserves the right to provide another substitute certified personal trainer for you.
第二十六条：会员与教练提前24小时预约如不能到场，须提前4小时致电或亲临俱乐部通知有关人员取消当天课程。否则，会员仍须交付该课程相关费用。
A cancellation notice 4-hour in advance is required for any sessions otherwise the member will be still charged for the missed session.
第二十七条：如果会员了解自己身体有任何关于身体方面的原因而导致不能使用本中心的任一器械或运动项目，请事先告知自己的私人教练。否则如果发生身体危险，或达不到会员所希望的目标，本俱乐部不负任何责任。
If you are aware that some of your physical conditions preventing you from using any facilities and training services in the club, please tell your PT in advance. Otherwise, if any dangers occur or you can not accomplish the anticipated targets, the club does not bear any responsibilities.
收银台为俱乐部唯一服务项目支付点，任何私自交易的教练指导性服务，本俱乐部不承担任何连带责任。
The member should only pay at the cashier counter. The club will not be responsible for any training service which traded in private.
第五章更衣柜及私人储物柜的使用管理规定 Locker and private locker
第二十八条：公共更衣柜和私人储物柜为寄存日常用品之用，不得存放现金、电脑、手机、贵重物品及任何食品，禁止存放易燃易爆及非法物品，如存放了贵重物品而丢失，本俱乐部将不负担赔偿责任。
Public lockers and private lockers are used to deposit daily-use articles and cannot be used to Deposit cash, computers, mobile telephones，Valuables and food. The explosives, flammable and illegal articles are strictly prohibited to be stored in any lockers in the club. The club will not be responsible for any loss of valuables in the lockers.
第二十九条：公共更衣室内的更衣柜为公共使用，会员无论任何原因离开俱乐部时，必须打开更衣柜并不得私自占用，本俱乐部有权在会员不在俱乐部时清理更衣柜并无法负责保管其中的物品。
Whenever the members leave the club for any reasons, they must keep the public lockers open. The club has right to clean the public lockers unoccupied and is not responsible for taking care of any articles left in them.
第三十条：会员如长期占用公共更衣柜，本俱乐部有权扣除其会员卡使用时间。
If the members occupy public lockers for long time, the club has the right to cut down the membership time.
第三十一条：会员租用的私人储物柜仅限在租赁协议规定有效时间内使用，会员需在私人储物柜到期前至前台或水吧办理续约手续。如不再续约需清空租赁柜并告知前台。同时会员卡到期，私人储物柜如有未使用月份将自动作废。
Private Lockers can only be used during period stated in the rental contract. Once the period has expired, the members must renew the rental contract at front desk and water bar. If not to renew it, the members have to empty the lockers and inform the front desk. The private locker rental contract will expire once upon the membership expires.
第三十二条：租赁柜到期一周后，如会员不再续约，本俱乐部将对租赁柜进行清理。并无法负责保管其中的物品。
If a rental locker is not vacated one week after the rental contract expires, the club will clear the lockers and the club will not be responsible for taking care of articles inside the lockers.
第三十三条：公共更衣柜手牌需会员保管好，如有丢失收取手牌成本费，私人更衣柜钥匙由会员自行保管，如有丢失收取钥匙成本费。

第三十四条：私人储物柜仅限会员本人使用，不得转借或转租。
Rental lockers are for current members only. It is forbidden to borrowed or rented to other person.
第三十五条：私人储物柜一经租赁后，若因会员单方面原因要求退租，俱乐部将扣除您的所有租金,并将押金无息返还。

第六章必须申报的情况和禁止的行为 Situations must be announced and behaviors must be forbidden
第三十六条：患有皮肤病、传染病、心脏病、高血压、糖尿病、肝、肾功能不良及其他不适合本俱乐部活动的疾病患者，入会前须向本俱乐部说明和申报以往病史，因隐瞒上述各类病史入会后在本俱乐部发生意外或加重病情的，后果由会员独自承担，本公司和本俱乐部概不负责。
People who suffer from skin disease, infectious diseases, heart disease and hypertension or any serious illnesses or afflictions must state to the club their health history. If members do not disclose their health history and suffer injury when using the facilities in the club, the club does not take any responsibilities.
第三十七条：吸食毒品者不得入会，因隐瞒吸食毒品而入会的，一经发现立刻取消会员资格，所有费用不予退还，造成的后果由其本人自行承担，本俱乐部概不负责。
Drug addicts and drug users are not welcome to join our clubs. If a member conceals their drug addiction , their membership will be cancelled immediately without refund or compensation. The club will not be liable for any loss.
第三十八条：饮酒者在体内酒精未分解挥发之前不得进入本俱乐部参加各项运动，因饮酒而强行进行活动发生意外的，责任由其自行承担，本公司和俱乐部不负任何责任。进入本俱乐部的人员不准携带和饮用含有酒精的饮料。
Members are not allowed to use the facilities if they are drunk or have consumed large amounts of alcohol. The club accepts no responsibility and is not liable for injuries cause because members are drunk. Anybody is forbidden to bring alcohols into the club.
第三十九条：会员在参加活动后即时将自己的私人物品带离俱乐部，本俱乐部不负保管义务，因会员未能及时将私人物品带走而发生遗失的，本俱乐部概不负责。
Members who take personal belongings into the club are wholly responsible for them. The club takes no responsibility for any loss of belongings within the club.
第四十条：定金保留有效期为五个工作日，过期将不予退还。在交纳定金时候必须指定办卡人员信息，定金不得转让他人使用。
Deposit payments have a validity of five working days. If this time is exceeded and the member has not paid the balance and joined the club, then the deposit can not be refunded. Transfers of deposits are not allowed.
第四十一条：会员需提前声明，身体状况良好，并无任何医学原因、损伤或残障阻碍，不影响使用本俱乐部所有设施及私教训练服务。
Members must state before they join the club they are in good health and are able to use all facilities and join in classes without any ill affects.
第四十二条：会员签署本协议后，本俱乐部不负责就会员使用设施及接受集体训练服务的身体状况及能力向会员提出医学意见。如会员在签署本协议之时或之后有任何健康方面的担忧，应在使用设施及进行私教训练之前咨询专业医生。
The club is not responsible for giving medical advice or suggestions about the member's health. If a member is concerned about their health after joining the club they must consult a medical physician/doctor.
第四十三条：会员在知悉或理应知悉自身存在任何可能阻碍自己使用俱乐部全部设施的健身问题，而仍签署了本协议，即表明会员同意本身的练习活动会因此受限制。然而由于会员选择签署本协议，仍需缴付有关的费用，犹如会员使用全部设施一样。
Members who sign the agreement and pay the membership fee should acknowledge they can use all of the facilities; members who have limitations on certain equipments will not be compensated.
第七章附则 Bylaws
第四十四条：会员不得在俱乐部内进行未经本俱乐部允许的任何收费类活动，包括但不限于对于指导会所内其他会员训练30分钟以上时间的情况。本俱乐部有权停止该会员的会籍期。
Members are not allowed to conduct any business what so ever within the club, especially personal training if members are seen training or guiding other members for longer than 30 minutes, otherwise they will have their membership terminated without refund.
第四十五条：本协议若经涂改或有手写条款，均视为无效，仍以本协议原始印刷文字为准。
Any changes or handwritten notes on the agreement will not be accepted and be invalid. The agreement is not to be changed in any way.
第四十六条：本协议具有约束力，并取代任何口头或书面协定。且法院宣布本协议任何部分无效，本协议其余部分将不受影响并继续有效。本俱乐部因任何原因而未能执行本协定下的任何权利，并不代表本俱乐部将放弃于日后执行有关权力的权利。
This agreement has binding force and substitutes any spoken or written agreement. If court announces that part of this agreement is not valid, other parts will not be affected. If the Club failed to carry out any right under this agreement, it does not mean that the club will give up carrying out these rights in the future.
第四十七条：此协议最终解释权归三毛健身所有。
The interpretation of this agreement shall be owned by the three hairs fitness.
